Double click execute bracket order executing at half size twice

Hi team,

Yesterday I executed on the risk reward tool with a 0,25% trade per the set bracket orders. As I have the double click feature on, I executed as per normal. Once I executed, I noticed that two positions were executed at half size as opposed to 1 full size trade. I have check to confirm that the double click feature is turned on and that my risk was set at the required 0,25% and both were as discussed above (no human error to my knowledge involved). 

Please advise as such.
